Hookwire Localisation

A hookwire localisation procedure is used to identify and mark a specific area of concern in the breast, usually before a surgical biopsy or excision.

The procedure involves the insertion of a fine wire into a targeted area in your breast. This wire guides the surgeon to the target and the wire and the lesion are then removed.

The radiologist will determine whether the procedure will be performed under ultrasound or mammographic guidance.

Hookwire Localisation FAQs

  • The purpose of hookwire localisation is to guide the surgeon to the exact location of an abnormality found on imaging studies. This ensures the targeted tissue can be accurately removed or biopsied.

    • The radiologist will use a local anaesthetic to help minimise any pain. Discomfort is usually minimal.  
    • The radiologist will insert the fine wire through a fine needle using either ultrasound or mammographic guidance.
    • The needle is removed and the fine wire remains in place through a small anchor hook on the end.
    • The portion of the wire which lays outside the breast is then taped down to the skin. 
    • The hookwire will be removed during your operation by the surgeon.
    • You will be admitted to the hospital or day surgery unit before your procedure at Queensland X-Ray.
    • We ask that you refrain from using deodorant, antiperspirant, talcum powder, perfume or lotions, as this can interfere with the quality of the images.
    • You may need to provide us with your previous mammograms and breast ultrasound images. If you had these exams with us, we will already have your images stored. The surgeon will have access to your images via our image and report viewing platforms.
  • Following the procedure, two gentle mammograms will be taken to show the surgeon the location of the wire/s. After your procedure the skin site will be cleaned and waterproof dressing applied. You will then be sent to the Day Unit to await your surgery. Risks and complications are rare.

  • A hookwire localisation procedure typically takes between 30 and 45 minutes.
