Have a referral but can’t upload it?

That’s okay as we will contact you to confirm your appointment. You’ll need to bring your referral with you on the day of your appointment.

Cards

To confirm your billing, we need to know if you have a Pension Card or a Health Care Card. Seniors cards are not accepted.
